Family Case Management is a program designed to provide comprehensive and holisitic services to qualified clients in Jefferson County. The target population is pregnant women and infants up to one year of age. Extended services are given to children above one year of age as deemed necessary.

Family Case Managers serve as a referral source for clients to local physicians for health care and social service agencies. Other services offered include Denver Developmental Screenings, Well Child Exams, Immunizations, and APORS follow-up (when needed).

General health education is given related to the three trimesters of pregnancy, along with infant and child care. Home assessments are done to provide infant and child safety tips.

Case Managers follow each client according to local protocols to ensure that services that are needed by the client are obtained. They work with the clients closely to develop a trusting relationship to better serve the client themselves.

Jefferson County Health Department also works with Illinois Department of Public Aid in getting pregnant women registered or signed up for prenatal medical card and infants and older children signed up for Kid Care.
